- Permanent Insomnia Curse: Beds only save your spawn point because you can never sleep again, though you will not have to worry about normal Phantoms
- Longer Day Cycle: Days are stretched to forty minutes of relative safety, while nights last twenty minutes of pure survival terror
- Sanity Mind Meter: Rain will slowly drain your sanity and make you lose your mind, but playing music discs on a jukebox will help you feel better
- Horde events trigger every five nights to send a massive wave of monsters straight to your base
- A helpful quest book guides you through chapters and rewards you with spikes and food for completing tasks
- Upgradable backpacks can be crafted with gold and leather, then expanded with iron for more inventory space
- Graveyards contain lootable sarcophagi that hold gold blocks or hidden monsters waiting to jump out
- Water monsters, fast hellhounds, and infected villagers hunt you down in the dark
- Creepers and explosive beasts will detonate instantly when they get within one block of you
